Job summary

Anchor Health is seeking an RN Case Manager to coordinate hospice care across patient homes and communities in California. The role involves conducting comprehensive assessments, developing and updating care plans, and collaborating with an interdisciplinary team to manage pain and symptoms.

Requires an active CA RN license and CPR certification; hospice experience preferred. Valid driver’s license and reliable transportation are needed for on-site/home visits.

Qualifications

• Hospice care experience preferred; at least 1 year.
• CPR certification required and maintained.
• Valid driver’s license, auto insurance, reliable transportation.

Responsibilities

• Completes initial, comprehensive, and ongoing assessments to determine hospice needs.
• Provides nursing care using nursing process and manages pain/symptoms.
• Develops a patient- and family-centered care plan and coordinates home care services.
• Initiates preventive and rehabilitative nursing procedures per physician plan.
• Acts as Case Manager and coordinates patient care for assigned caseload.
• Documents clinical notes and tracks pain/symptom changes.
