Job summary

Central Coast VNA & Hospice is seeking an RN Hospice Case Manager to coordinate and deliver skilled nursing care and support services in patients’ homes across the Central Coast. You’ll collaborate with physicians and the care team to ensure comfort, safety, and quality of life for patients and families.

The role requires California RN licensure and at least one year of Home Health or Hospice experience, with CPR certification and strong teamwork abilities.

Qualifications

• Active Registered Nurse license in the California.
• Minimum of one year nursing Home Health or Hospice experience required.
• Knowledge of home health, hospice, palliative care, and other agency services preferred.
• Current CPR certification.
• Ability to work collaboratively with physicians, families, caregivers, and the interdisciplinary team members.

Responsibilities

• The Hospice Case Manager collaborates with physicians and the interdisciplinary care team to identify, coordinate, and address the home care needs of patients, families, and caregivers.
• The Hospice Case Manager provides skilled nursing care and supportive services using the nursing process, focusing on patient comfort, safety, and quality of life.
• Coordinates referrals to other disciplines and services and facilitates transitions between care levels as needed.
• Manages patient care in accordance with agency standards, regulations, and payer requirements.
• Provides education and support to patients, families, and caregivers and prepares patients and families for discharge when appropriate.
