Qualifications

• Requires an Associate degree in Nursing.
• Prefer a Bachelor’s degree in Nursing.
• RN license and CPR as applicable by state.
• Hospice care experience preferred.

Responsibilities

• Complete initial patient and family assessment to determine hospice needs.
• Provide a full nursing assessment and regular re-evaluation of patient needs.
• Develop and revise care plans with nursing diagnoses and goals.
• Administer medications and treatments as prescribed by the physician.
• Counsel patient and family on nursing needs and care plans.
• Participate in IDT meetings and case conferences.
