Job summary

Salus Hospice in Orange County is seeking a compassionate Hospice RN Case Manager to join our dedicated team serving Central and Southern Orange County. You will work with the Interdisciplinary Team to assess patient needs and coordinate hospice services in various settings.

The ideal candidate holds a CA RN license, has hospice experience, and demonstrates excellent interpersonal skills, reliability, and a patient-centered approach to end-of-life care.

Qualifications

• Registered Nurse with CA state license.
• Minimum one year of hospice RN experience.
• Strong background in medical/surgical, ICU/CCU, or ER is helpful.
• Valid driver's license, auto insurance, and reliable transportation.
• Excellent interpersonal skills and passion for hospice care.
• Understanding of hospice philosophy and terminal illness needs.
• Self-directed with ability to work with little supervision.
• Current physical and TB test results.

Responsibilities

Hospice RN Case Manager Responsibilities

• Perform RNduties on hospice team.
• Serve as a RN Case Manager for the hospice Interdisciplinary Team
• Assess for signs and symptoms of discomfort including presence of pain and takes prompt and appropriate action
• Establishes plan of care while collaborating and communicating with the Interdisciplinary Team including physician when appropriate
• Coordinates all hospice services provided to patients/families in all settings to ensure quality of care

Ideal Hospice RN Case Manager Candidate Background

• Registered Nurse with valid CA state license
• Minimum one year of hospice RN experience
• Strong background in medical/surgical, ICU/CCU, or ER is helpful
• Valid driver’s license, auto insurance, reliable transportation and willingness to drive to patients place of residence
• Must have excellent interpersonal skills and a passion for hospice
• Understanding of hospice philosophy and needs of the terminally ill
• Self-directed with ability to work with little supervision
• Current Physical and TB test results
