Job summary

Salus Hospice in Orange County is urgently seeking a compassionate Hospice RN Case Manager to join our team. This full-time role covers Central and Southern Orange County and requires a CA RN license and at least one year of hospice RN experience.

You will collaborate with an Interdisciplinary Team to assess patient needs, establish care plans, and coordinate services to ensure high-quality hospice care. A valid driver’s license and reliable transportation are required.

Qualifications

• Registered Nurse with CA state license.
• Minimum one year hospice RN experience.
• Background in medical/surgical, ICU/CCU, or ER is helpful.
• Must have physical and TB test results.
• Willingness to drive to patients' residences.

Responsibilities

• Perform RN duties on the hospice team.
• Serve as an RN Case Manager for the Interdisciplinary Team.
• Assess for signs and symptoms of discomfort and manage pain.
• Establishs plan of care with the Interdisciplinary Team, including physician when appropriate.
• Coordinates all hospice services to ensure quality of care across settings.

Hospice RN Case Manager Responsibilities:

• Perform RNduties on hospice team.
• Serve as a RN Case Manager for the hospice Interdisciplinary Team
• Assess for signs and symptoms of discomfort including presence of pain and takes prompt and appropriate action
• Establishes plan of care while collaborating and communicating with the Interdisciplinary Team including physician when appropriate
• Coordinates all hospice services provided to patients/families in all settings to ensure quality of care

Ideal Hospice RN Case Manager Candidate Background:

• Registered Nurse with valid CA state license
• Minimum one year of hospice RN experience
• Strong background in medical/surgical, ICU/CCU, or ER is helpful
• Valid driver’s license, auto insurance, reliable transportation and willingness to drive to patients place of residence
• Must have excellent interpersonal skills and a passion for hospice
• Understanding of hospice philosophy and needs of the terminally ill
• Self-directed with ability to work with little supervision
• Current Physical and TB test results
