Job summary

Gentiva is seeking a dedicated Hospice RN / RN Case Manager to lead patient care at home, guiding families through end‑of‑life planning with empathy and expertise. You will assess patients, manage symptoms, and coordinate with physicians, social workers, chaplains, and other professionals to tailor plans of care that respect patient goals.

The role requires strong clinical judgment, compliance with regulatory standards, and compassion for patients and families during difficult times.

Qualifications

• Current RN license in Tennessee or state of employment.
• Minimum of one year experience as an RN in hospice, palliative care, or home health.
• Hospice or palliative care experience preferred.

Responsibilities

• Perform comprehensive patient assessments and reassessments with education for patients, families and caregivers.
• Deliver nursing interventions and symptom management aligned with goals of care.
• Develop and update individualized plans of care and determine visit frequency.
• Coordinate care across IDG including physicians and other staff.
• Educate patients and caregivers on disease, self-care, end-of-life care, medications and nutrition.
• Communicate changes in condition to the care team for timely interventions.
• Support transitions and referrals across home health, palliative and hospice levels.
• Document timely clinical notes, orders, progress, and care plans.
• Maintain strong relationships with the IDG and participate in on-call activities.
