Job summary

Gentiva Hospice is seeking a dedicated Registered Nurse to serve as an RN Case Manager in patient homes across the community. You will perform comprehensive assessments, implement and adjust plans of care, and collaborate with an interdisciplinary team to ensure compassionate, high‑quality end‑of‑life care.

You will educate patients and families on disease progression, symptom management, and medications, while maintaining precise documentation and adherence to regulatory standards.

Qualifications

• RN license required in state of employment.
• Minimum 1 year RN experience in hospice, oncology, home health, or related.
• Hospice or palliative care experience preferred.

Responsibilities

• Perform comprehensive patient assessments and reassessments including in‑person/telephonic RN assessments and education to patients and caregivers.
• Deliver skilled nursing interventions and symptom management per goals of care.
• Develop and update individualized plans of care and visit frequency.
• Coordinate care across IDG and consult with physicians and nurse practitioners.
• Educate patients and caregivers on disease, care, medications, nutrition and dietary needs.
• Document timely clinical records and maintain infection control compliance.
