Job summary

Gentiva is seeking a compassionate Hospice RN / RN Case Manager to lead patient care at home or in preferred settings. You will assess, plan, and deliver skilled nursing with a focus on comfort, dignity, and family support, coordinating with an interdisciplinary team to refine care plans.

You will educate families, document care, and participate in on-call coverage while ensuring regulatory compliance and high-quality end-of-life care.

Qualifications

• Current RN license in the state of employment.
• CPR certification.
• Driver's license and automobile insurance.

Responsibilities

• Perform comprehensive patient assessments and reassessments including in-person/telephonic RN assessments and education for patients, families and caregivers.
• Deliver skilled nursing interventions and symptom management aligned with goals of care.
• Develop and update individualized plans of care including visit frequency to meet needs.
• Coordinate care across interdisciplinary teams including physicians and other professionals.
• Educate patients and caregivers about disease process, self-care, end-of-life care, medications and dietary needs.
• Communicate changes in condition to the appropriate provider.
• Support continuity of care and transitions between home health, palliative and hospice levels.
• Document timely clinical records and maintain infection control standards.
• Maintain relationships with the IDG and involve team as needed.
• Participate in on-call and patient support activities as applicable.
