Job summary

Elara Caring is seeking a Hospice Registered Nurse Case Manager to join our team and support patients in their homes. The role centers on assessing needs, coordinating care and educating families as part of an interdisciplinary plan of care.

The position requires an active RN license and a minimum 1 year of clinical nursing experience, with travel between patient homes. We offer comprehensive benefits and a focus on compassionate end‑of‑life care.

Qualifications

• Current State license as a Registered Nurse
• Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing
• 1+ year of experience as a nurse in a clinical setting
• Ability to lift, push, pull, and support up to 50 pounds
• Valid driver’s license or reliable transportation for travel between patient homes
• Hospice or home health experience preferred

Responsibilities

• Assess hospice patients and families to identify needs across physical, psychological, social, spiritual and environmental domains
• Monitor patient condition and communicate significant changes to the physician and care team
• Educate and support patients, families, caregivers and facility staff in the Plan of Care
• Provide education on hospice philosophy, pain and symptom management to staff in various care settings
• Provide compassionate support at end of life and complete bereavement assessments
