Job summary

Elara Caring is seeking a Registered Nurse Case Manager to support hospice patients and families, coordinate care across settings, and educate staff and caregivers. The role emphasizes compassionate support, timely communication, and adherence to the interdisciplinary Plan of Care.

Qualified candidates hold an active RN license in the state, have 1+ year clinical experience, and are willing to travel to patient homes. Driver’s license or reliable transportation is required.

Qualifications

• Current State license as a Registered Nurse.
• Graduate of an accredited nursing program (Certificate, Diploma, Associate, or Baccalaureate).
• 1+ year of nursing experience in a clinical care setting.
• Ability to frequently lift, push, pull, and support up to 50 pounds.
• Must possess a valid driver's license or reliable transportation to travel to patient homes.
• Hospice or home health experience preferred.

Responsibilities

• Assess hospice patients and families to identify physical, psychological, social, spiritual, and environmental needs.
• Monitor patient condition and promptly communicate significant changes to the physician and care team.
• Educate and support patients, families, caregivers, and facility staff in following the interdisciplinary Plan of Care.
• Provide education on hospice philosophy, pain management, and symptom management to staff in various care settings.
• Provide compassionate support at the time of death and complete bereavement assessments for families and caregivers.
