Job summary

INTEGRIS Health in Oklahoma City, OK is seeking a Day Shift, full-time RN Case Manager for Hospice. You will coordinate patient care, collaborate with an interdisciplinary team, and ensure the plan of care aligns with physician orders and patient goals.

Requirements include 1 year of clinical nursing experience, an Associate's or Bachelor's degree, and active RN licensure in Oklahoma (or multi-state eNLC) with BLS. Strong communication in English is essential.

Qualifications

• 1 year of clinical nursing experience required.
• Associate's or Bachelor's degree in nursing.
• RN licensure in Oklahoma or a current multi-state license (eNLC).
• BLS certification issued by American Red Cross or American Heart Association.

Responsibilities

• Coordinate all aspects of service delivery as defined by physician orders.
• Participate in interdisciplinary group meetings; manage plan of care and reevaluate as needed.
• Ensure implementation of activities for improved patient care quality.
• Supervise Home Health Aide activities and plan of care.
• Educate nursing/medical students during rotations.
• Handle pain and symptom management within patient care.
