Job summary

The role requires an active Oklahoma RN license, CPR certification, and 1+ year of clinical experience. The position offers full-time hours, with travel within a 60-mile radius, and a focus on comprehensive patient care and documentation.

Qualifications

• Active Oklahoma RN license required.
• 1+ year of clinical experience preferred.
• CPR certification required or ability to obtain within 90 days of hire.

Responsibilities

• Make the initial nursing evaluation to determine eligibility for hospice services within 48 hours of referral.
• Identify the patient’s/family’s physical, psychosocial, and environmental needs, reassessing no less than every 15 days.
• Initiate and coordinate the plan of care.
• Document problems, goals, interventions, and patient/family responses to hospice care.
• Collaborate with the patient/family, attending physician, and other members of the IDG in daily care.
• Instruct and supervise the patient/family in self-care techniques when appropriate.
• Maintain accurate and relevant clinical notes regarding the patient’s condition.
• Function in any home situation regardless of the client’s age, race, creed, color, sex, disability, or financial condition.
