INTEGRIS Health in Oklahoma City, OK is seeking a Day Shift, full-time RN Case Manager for Hospice. You will coordinate patient care, collaborate with an interdisciplinary team, and ensure the plan of care aligns with physician orders and patient goals.

Requirements include 1 year of clinical nursing experience, an Associate's or Bachelor's degree, and active RN licensure in Oklahoma (or multi-state eNLC) with BLS. Strong communication in English is essential.

Job Description

Join our team as a day shift, full time, RN Case Manager - Hospice, at INTEGRIS Health, in Oklahoma City, OK.

Responsibilities

• Manages/coordinates all aspects of service delivery as defined by physician orders
• Participates in interdisciplinary group meetings/re-certification process. The Case Manager is directly responsible for all aspects of the patient's care. The plan of care is discussed with all the disciplines and reevaluated as needed. This includes needs such as comfort measures, pain management, psychosocial needs, spiritual needs, volunteer needs and/or bereavement issues
• Ensures implementation of activities for improved quality of patient care
• Supervises Home Health Aide activities and plan of care
• Supervises and educates nursing/medical students during their rotation
• Specializes in pain and symptom management issues in patient care The Case Manager Hospice reports to Manager Hospice. This position may have additional or varied physical demand and/or respiratory fit test requirements. Potential for exposure to infections and communicable diseases, blood and body fluids, electrical equipment, chemicals. Must follow standard precautions. Qualifications

REQUIRED QUALIFICATIONS

EXPERIENCE:

• 1 year of clinical nursing experience

EDUCATION:

• Associate's or Bachelor's degree
• Graduation from an approved school of nursing

LICENSE/CERTIFICATIONS:

• RN (Registered Nurse) Current licensure as a Registered Nurse (RN) in the State of Oklahoma or current multistate license from a Nurse Licensure Compact (eNLC) member state
• BLS (Basic Life Support) Issued by American Red Cross or American Heart Association within 30 days of hire

Must be able to communicate effectively in English (verbal/written). This job requires the incumbents to operate a INTEGRIS-owned vehicle OR personal vehicle (non INTEGRIS-owned) and therefore must have a current Oklahoma State Drivers License as well as a driving record which is acceptable to our insurance carrier.
