A Path of Care Hospice in Norman, OK is seeking a full-time RN Case Manager to provide compassionate, high-quality nursing care to patients in their homes. Qualifications

• Active Oklahoma RN license.
• 1 year clinical experience.
• Reliable transportation and vehicle liability insurance.
• Hospice/home health/critical care or emergency room experience preferred.
• Strong interpersonal and communication skills.
• Ability to adapt to changing circumstances, mentor others, and work with compassion and patience.

Responsibilities

• Complete initial assessments within 48 hours of hospice election.
• Collaborate with attending physician and interdisciplinary team to complete comprehensive assessments.
• Develop, coordinate, and initiate individualized plans of care.
• Conduct ongoing assessments and update care plans as patient condition changes.
• Address physical, psychological, social, and spiritual needs.
• Communicate regularly with physicians and care team to maintain continuity of care.
• Maintain accurate, timely documentation to meet regulatory requirements.
• Provide leadership by instructing caregivers and supervising LPNs and aides.
• Practice infection control and report signs of abuse.
• Provide direct patient care as needed and assist with discharge planning.
• Participate in professional development and agency initiatives.

What You'll Do as an RN Case Manager

As an RN Case Manager, you ensure patients receive exceptional nursing care and support throughout their hospice journey. Your responsibilities include:

Qualifications
