Job summary

St. Croix Hospice is seeking a Registered Nurse Case Manager to deliver holistic in-home hospice care. You will assess patient needs, develop care plans with families, administer medications, and guide families through end-of-life conversations with empathy.

You will collaborate with a care team, rotate on-call duties, and ensure smooth transitions while maintaining quality of life, dignity, and comfort for patients and their loved ones in Des Moines metro area.

Qualifications

• Associates, Bachelors, or Masters of an accredited nursing program (RN)
• 1+ years of recent acute care experience required; hospice/home health experience preferred

Responsibilities

• Complete initial and ongoing comprehensive assessments to determine hospice needs
• Develop individualized care plans with the patient and family, updating as needs evolve
• Administer medications and treatments as prescribed
• Educate patients and caregivers on care techniques, medications, diet, and symptom management
• Rotate on-call duties and ensure seamless care transitions
