Job summary

Magnolia Hospice in Austin is seeking a Registered Nurse Case Manager (RNCM) for a full-time position. The RNCM will administer hospice care in patients' homes or facilities, coordinate with the care team, and ensure a comprehensive Plan of Care is followed.

Responsibilities include patient assessments, education for patients and families, and on-call coverage as needed. Requires RN license, CPR, and hospice experience preferred. Travel within service area may be required.

Qualifications

• Graduate of an accredited school of nursing.
• Two (2) years of experience as a Registered Nurse (RN) in a clinical setting preferred hospice.
• Current Valid State Nursing license (RN), CPR certification required.
• Driver's license and proof of auto liability insurance; no listing in the OIG Excluded Provider listing.

Responsibilities

• Administers hospice care to clients in their residence or facilities.
• Coordinates care with the multidisciplinary team and refers as needed.
• Develops and implements a written Plan of Care based on physician orders and agency guidelines.
• Documents patient status, interventions, and outcomes accurately and timely.
• Educates patients and families on care plans, symptom control, and end-of-life options.
• Participates in on-call rotation and makes scheduled visits.
