Job summary

Hospice Austin is seeking an experienced RN Case Manager to provide comprehensive nursing care in homecare and facility settings in the Austin area. You will conduct admissions visits, assess patients, develop and update plans of care, manage symptoms, and coordinate with the interdisciplinary team to ensure high-quality end-of-life care for patients and families.
