Job summary

Magnolia Hospice - Arlington is seeking an experienced RN Case Manager to provide comprehensive hospice care in patients' homes or facilities. The role involves coordinating with an interdisciplinary team, developing and implementing care plans, and educating families on end-of-life care.

The successful candidate will hold a nursing license, CPR certification, and have two years of RN experience (hospice preferred).

Qualifications

• Education: Graduate of an accredited school of nursing.
• Experience: Two (2) years of RN clinical experience; hospice preferred.
• License / Certification: Current valid RN license; CPR certification required.
• Drive: Driver's license and auto liability insurance; no OIG exclusions.

Responsibilities

• Administers hospice care to clients in their residence or facilities.
• Coordinates care with an interdisciplinary team and family.
• Makes written plan of care and documents nursing interventions.
• Educates patients and families on care plans and end-of-life options.
• Participates in on-call rotation and scheduled visits.
• Maintains confidentiality and complies with laws and policies.
• Develops and implements individualized care plans with team input.
