Job summary

Magnolia Hospice in San Marcos is seeking an RN Case Manager for full-time, coordinating care for patients at home or in facilities. You will work with a multidisciplinary team to develop and implement individualized plans of care, ensuring timely documentation and patient education.

The role requires RN licensure, CPR, and willingness to travel locally. You will manage medications and support patients and families through end-of-life care with compassion and adherence to policy.

Qualifications

• Education: Graduate of an accredited school of nursing.
• Two years RN experience, hospice preferred.
• Current valid RN license and CPR certification.
• Driver's license and auto liability insurance; no OIG exclusion.

Responsibilities

• Administers medications and treatments per Agency policy.
• Develops and documents Plan of Care with patient and family.
• Documents patient status accurately and timely.
• Collaborates with the interdisciplinary team to implement individualized care plans.
• Makes visits as scheduled and participates in on-call rotation as required.
• Maintains confidentiality and complies with healthcare regulations.
