Job summary

Qualifications

• RN license in good standing (state of Texas).
• CPR certification required.
• Driver’s license and auto liability insurance.
• No listing in OIG Excluded Provider listing.

Responsibilities

• Administer hospice care for clients in residence, skilled facilities, or assisted living.
• Coordinate care with multidisciplinary team and family; assume planning responsibility.
• Perform baseline nursing assessments and document care plans.
• Administer medications and treatments per policy and physician orders.
• Educate patients and families on plan of care and symptom control.
• Participate in on-call rotation and travel locally as needed.
• Maintain confidentiality and comply with healthcare regulations.
