Summa Health at Home and Hospice is seeking a Registered Nurse to join our Akron-area team. Expect a Mon-Fri schedule with rotating weekends, visits to patients in home settings, and close collaboration with physicians, social workers, and aides to deliver comprehensive end-of-life care.

The role requires an active RN license in the state of employment and at least one year of RN experience in acute or home health settings, with a compassionate approach and strong documentation skills.

Responsibilities

• Oversees overall patient care from beginning to end of life.
• Performs initial, comprehensive, and ongoing skilled nurse visits as per plan of care.
• Updates doctor orders in patient chart; notifies doctor of changes.
• Manages interdisciplinary care and provides end of life care at home.
• Administers medications and ensures continuity of care during admissions, transfers and discharges.
