Job summary

Summa Health at Home and Hospice seeks an experienced Registered Nurse Case Manager to coordinate end-of-life care in patients' homes across Akron area.

The RNCM will conduct initial and ongoing skilled nursing visits, update physician orders, and oversee interdisciplinary care with physicians, nurses, and family members. A compassionate, patient-centered approach and ability to manage schedules is required.

Qualifications

• Active RN license in the state of employment.
• Hospice experience is preferred.
• One year of RN experience in acute or home care.
• Detail-oriented, calm, patient, and team-oriented.
• Must have valid driver's license and auto insurance.

Responsibilities

• Oversees the overall care of the patient from beginning to end of life.
• Performs initial and ongoing skilled nursing visits.
• Updates doctor orders and notifies changes in patient condition.
• Coordinates interdisciplinary care and counsels patient and family.
• Documents treatment and education per agency protocols.
