Job summary

Summa Health at Home and Hospice in Akron is seeking a Registered Nurse Case Manager (RNCM) to oversee end-of-life care for patients at home. You will perform initial and ongoing skilled visits, update orders, and coordinate with physicians, nurses, and aides to deliver high-quality, cost-effective care.

Qualifications

• One year of RN experience in acute care or home care is required.
• Hospice experience is preferred.
• Maintains active RN license in the state of employment.
• Ability to follow agency policies and provide compassionate care.
• Detail-oriented with strong communication and teamwork.

Responsibilities

• Oversees overall patient care from start to end of life.
• Performs initial, comprehensive, and ongoing skilled visits.
• Updates doctor orders in patient chart and notifies changes.
• Manages interdisciplinary care and coordinates with physicians.
• Administers medications as prescribed and documents treatments.
• Supervises nurses aides and LPNs; ensures appropriate admissions/discharges.
• Provides end-of-life care at home and participates in on-call and case conferences.
• Maintains documentation per Medicare/federal guidelines and agency policy.
