Job summary

Summit Home Care & Hospice in Dublin, OH is seeking a Registered Nurse (RN) to plan, organize and direct home care services. The nurse will assess patients, develop and revise care plans, and coordinate with physicians and families to deliver high‑quality community health care.

Responsibilities include initial assessments, ongoing nursing care, medication administration, on‑call duties, and acting as case manager for assigned caseload. Strong communication and documentation skills are essential.

Qualifications

• Associate or bachelor’s degree in nursing from an accredited school.
• A valid/active RN license.
• Experience in home health care or acute care preferred.
• Excellent observation, verbal and written communication skills, problem solving skills; nursing skills per competency checklist.

Responsibilities

• Complete an initial assessment of patients and family to determine home care needs.
• Provide a complete physical assessment and history of current and previous illnesses.
• Regularly re-evaluate patient nursing needs and revise care plan as patient status changes.
• Administer medications and treatments as prescribed by the physician.
• Act as Case Manager and coordinate patient care for assigned caseload.
• Prepare clinical notes and update primary physician at least every 60 days.
• Communicate with physician and community health professionals to coordinate care.
• Participate in on-call duties and ensure necessary items and services are available.
