Job summary

Traditions Health is seeking an experienced RN Case Manager to coordinate admissions and nursing care for patients in their homes. You will collaborate with the Interdisciplinary Group/Team, patients, families, and referring agencies to ensure comprehensive, person-centered care.

The role requires two years of RN clinical experience and at least one year in hospice, with current RN licensure and driving eligibility.

Qualifications

• Two years RN experience in a clinical care setting.
• One year full-time hospice experience.
• Current State RN license and valid driver’s license.
• Knowledge of state/federal regulations; strong interpersonal skills.

Responsibilities

• Assess hospice patients using LCD criteria to identify needs.
• Develop and document the plan of care in correlation with the assessment.
• Provide nursing care utilizing infection control measures (OSHA).
• Coordinate with Interdisciplinary Group/Team to meet patient/family needs.
• Communicate with attending physicians and hospice physicians involved in care.
• Timely submission of all required paperwork.
• Participate in hospice-sponsored in-service programs and QAPI program.
• Promote hospice philosophy and policies.
• Meet productivity standards.
• Provide on-call services as assigned.
• Maintain reliable transportation and auto liability insurance.
