Ennoble Care is hiring a Full-time Hospice Registered Nurse Case Manager to work within and around hospitals in Gwinnett County, GA and surrounding areas. The RN Case Manager collaborates with the director of nursing and interdisciplinary team to plan and deliver skilled nursing care maximizing comfort and health for patients and families.

Qualifications

• GA RN license required.
• 1–2 years nursing experience, hospice preferred.
• Ability to assess needs of patients and caregivers in varied settings.
• Skilled in nursing practice and report writing.
• Must be a team player within an interdisciplinary team.

Responsibilities

• Provide hospice nursing care for General Inpatient (GIP) level of care.
• Perform comprehensive nursing assessments for pain, dyspnea, agitation, and other symptoms.
• Develop, implement and update individualized care plans with the hospice team.
• Administer medications and treatments per physician orders.
• Evaluate and document patient responses and condition changes.
• Educate patients and families about dying process and symptom management.
• Coordinate care with hospital, SNF, or hospice inpatient staff.
• Participate in discharge planning and care transitions.
• Maintain timely clinical documentation of interventions and outcomes.
