Job summary

Gentiva Hospice is seeking a dedicated Hospice RN / RN Case Manager to provide skilled nursing care in patients' homes or other settings. You will assess, plan, and coordinate end-of-life care in collaboration with an interdisciplinary team, while educating patients and families and ensuring compassionate support.

Ideal candidates have 1 year of RN experience in hospice/home health and a strong commitment to patient-centered care and regulatory compliance.

Qualifications

• Must hold RN license in the state of employment.
• Minimum of 1 year RN experience in acute care, hospice or home health.
• Hospice or palliative care experience preferred.

Responsibilities

• Perform comprehensive patient assessments and reassessments.
• Deliver nursing interventions and symptom management per Plan of Care.
• Develop and update individualized care plans with visit frequency.
• Coordinate care with interdisciplinary team including physicians and nurses.
• Educate patients and caregivers on disease, care, and end-of-life options.
• Document timely clinical records and maintain regulatory compliance.
