Optimal Hospice is seeking a Registered Nurse Case Manager in the Denver, Colorado area. You will perform comprehensive patient assessments, develop and manage care plans, and coordinate interdisciplinary care for hospice patients and families.

Ideal candidates will hold an RN license, NLN-accredited education, CPR certification, and a valid driver’s license, with hospice or home health experience preferred. The role emphasizes compassionate, patient-centered care and teamwork.

Qualifikationen

• RN with current licensure to practice in the state
• Graduate of NLN-accredited nursing program
• CPR certification current
• Valid driver’s license

Aufgaben

• Completes initial, comprehensive and ongoing assessments of patients and families to determine hospice needs.
• Provides professional nursing care utilizing all elements of the nursing process.
• Develops and revises plan of care with patient and family involvement.
• Administers medications and treatments as prescribed by the physician.
• Counsels patient and family in meeting nursing and related needs.
• Acts as Case Manager coordinating patient care for assigned caseload.
• Maintains accurate clinical notes and documents changes in pain/symptom management.
