Qualifications

• RN licensure in good standing in Colorado.
• Current CPR certification.
• Hospice nursing experience preferred.
• Ability to coordinate care and communicate clearly.

Responsibilities

• Completes initial and ongoing patient assessments and develops plans of care.
• Provides professional nursing care using the nursing process.
• Manages care plans with patient and family involvement.
• Administers medications and treatments as prescribed.
• Acts as Case Manager to coordinate patient care for assigned caseload.
• Maintains accurate clinical notes and communicates changes to physicians.
• Educates patient and family on self-care and care plans.
• Collaborates with interdisciplinary team to meet emotional and physical needs.
