Job summary

Optimal Hospice in Colorado seeks a Registered Nurse Case Manager to deliver compassionate hospice care. You will complete comprehensive patient assessments, develop and implement care plans, and coordinate with an interdisciplinary team to ensure comfort and quality of life for patients and families.

Qualifications

• Registered nurse with current licensure to practice professional nursing in the state.
• Graduate of National League for Nursing accredited school of nursing.
• Maintains a current CPR certification.
• Current valid driver’s license.
• At least one year of Hospice experience (preferred).

Responsibilities

• Completes initial, comprehensive and ongoing assessments of patients and families to determine hospice needs.
• Provides professional nursing care by utilizing all elements of the nursing process.
• Assesses and evaluates patient’s status by writing and initiating plan of care; regularly re-evaluating patient and family/caregiver needs.
• Uses health assessment data to determine nursing diagnosis.
• Develops a care plan that establishes goals based on nursing diagnosis and includes patient and family in planning.
• Initiates preventive and rehabilitative nursing procedures; administers medications and treatments as prescribed.
• Counsels patient and family in meeting nursing and related needs.
• Provides health care instructions to the patient per assessment and plan.
• Assists with activities of daily living and promotes self-sufficiency.
• Acts as Case Manager and coordinates patient care for the caseload.
• Documents clinical notes and reports changes in condition; communicates with physician.
• Coordinates with community health professionals to implement the plan of care.
• Educates patient and family on self-care and health counseling; works with interdisciplinary team.
• Maintains a safe environment for patients.
• Supports continuity of care with family/caregiver and team members.
• Attends interdisciplinary group meetings.
