Job summary

Optimal Hospice seeks a Registered Nurse Case Manager in Colorado to provide comprehensive in-home and facility-based hospice services to patients and families. This is a full-time position focused on assessment, care coordination and compassionate, patient-centered care.

You will conduct initial and ongoing assessments, develop and revise plans of care, administer medications as prescribed, educate families, and collaborate with an interdisciplinary team to ensure comfort and quality of life.

Qualifications

• Licensed Registered Nurse (RN) in good standing.
• Graduate of National League for Nursing accredited school of nursing.
• Current CPR certification.
• Valid driver’s license.
• Hospice experience preferred; strong assessment and communication skills.

Responsibilities

• Completes initial, comprehensive and ongoing patient assessments and family needs.
• Provides professional nursing care using the nursing process.
• Develops and revises care plans with patient and family involvement.
• Administers medications and treatments as prescribed by the physician.
• Coordinates patient care as Case Manager with interdisciplinary team.
