Job summary

Luminary Hospice is seeking a PRN Registered Nurse Case Manager to plan, organize, and direct hospice care in patients' homes and communities. The role emphasizes community health nursing and collaboration with an interdisciplinary team to meet patient and family needs.

Responsibilities include comprehensive assessments, care planning, medication administration, and ongoing education for patients and families. On-call duties and flexible scheduling are part of the role.

Qualifications

• Graduate of an accredited nursing program with current State licensure.
• CPR certification required; one to two years home care or acute care experience preferred.
• Bachelor’s degree; hospice care experience preferred.
• Valid driver's license and reliable vehicle.
• Excellent communication, observation, and nursing skills; able to lift up to 50 lbs.
• Must be able to work as PRN and manage patient care in home settings.

Responsibilities

• Perform initial and ongoing comprehensive patient and family assessment.
• Develop and revise care plans with the interdisciplinary team.
• Administer medications as prescribed and provide patient/caregiver education.
• Coordinate home-based hospice services and communicate with physicians.
• Participate in on-call duties and ensure equipment availability.
• Maintain accurate clinical notes and documentation.
