Job summary

Paycom - ATS is seeking a Hospice Registered Nurse Case Manager to provide professional nursing care to home care patients under physician direction, and in compliance with the State Nurse Practice Act and agency policies.

You will perform admissions, routine visits, on-call duties, and supervise LPNs while collaborating with an interdisciplinary team to ensure compassionate care across hospice services. Travel and accurate documentation are required.

Qualifications

• Must hold a current RN license in Oklahoma.
• Two years hospice nursing experience preferred.
• Certification in Hospice and Palliative Nursing desirable.
• Excellent verbal, written, and electronic communication skills.
• Ability to adapt to frequently changing tasks and priorities.
• Must possess a valid driver’s license and reliable transportation.
• Maintain accurate, timely documentation meeting Medicare and agency standards.

Responsibilities

• Provide direct nursing care to hospice patients in home and facility settings.
• Conduct admissions, routine skilled nursing and supervisory visits, hospice evaluations, on-call duties, and PRN visits.
• Maintain and update patient medical records per agency standards.
• Collaborate with an interdisciplinary team and participate in meetings and case conferences.
• Engage in ongoing education and training to stay current on treatments and practices.
• Perform supervisory visits for LPNs as assigned.
• Assist in onboarding and training new employees.
• Adhere to policies, procedures, and Medicare guidelines; ensure timely documentation and reporting.
