Job summary

Paycom - ATS is seeking an RN Case Manager for Hospice to provide skilled nursing care in patients' homes under physician orders. Active RN licensure in the state is required, with 1 year of nursing experience preferred and CPR certification.

You will evaluate patients, develop and adjust plans of care with the IDT, document thoroughly, and coordinate with physicians, caregivers, and providers while teaching and supervising staff per agency policy.

Qualifications

• Graduate of an approved school of professional nursing and licensed in practicing state.
• 1 year nursing experience preferred.
• Ability to exercise initiative and independent judgment.
• Acceptance of philosophy and goals of this Agency.

Responsibilities

• Provides services per the plan of care and completes the initial evaluation visit.
• Coordinates with IDT members; revises the care plan as patient needs change.
• Documents and completes OASIS assessments, reassessments, transfers, and discharge summaries as required.
• Supervises LPN/LVN every 30 days and HHA/CNA every 14 days per regulations.
• Counsels patient and family; maintains confidentiality and adheres to agency policies.
