Job summary

Elara Caring seeks a Registered Nurse Case Manager in Enid, Oklahoma to coordinate hospice patient care and educate families. You will assess needs, monitor conditions, and collaborate with physicians to implement the interdisciplinary Plan of Care.

A valid RN license and 1+ year of clinical nursing experience are required, with travel between patient homes expected. Comprehensive benefits are provided.

Qualifications

• Current State license as a Registered Nurse.
• Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing.
• 1+ year of experience as a nurse in a clinical care setting.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Must possess a valid driver's license, or have reliable transportation to travel between patient homes and assigned work locations.
• Experience in hospice or home health environment is preferred.

Responsibilities

• Assess hospice patients and families to identify physical, psychological, social, spiritual, and environmental needs.
• Monitor patient condition and promptly communicate significant changes to the physician and care team.
• Educate and support patients, families, caregivers, and facility staff in following the interdisciplinary Plan of Care.
• Provide education on hospice philosophy, pain management, and symptom management to staff in nursing homes, hospitals, and other care settings.
• Provide compassionate support at the time of death and complete bereavement assessments for families and caregivers.
