Job summary

Kinder Hearts Home Health and Hospice seeks a registered nurse to plan, organize and direct home care services in patients' homes and communities. The role emphasizes community health education and outcomes in a patient-centered care model.

The nurse collaborates with physicians, patients and families to develop and implement care plans, monitor progress, and coordinate with the care team to meet goals for safe home living.

Qualifications

• Must graduate from an approved school of nursing and be licensed in Texas.
• Minimum of two years of experience, preferably in public health or home care.
• Current CPR certification.
• Valid driver's license and reliable vehicle.
• Self-directed and able to work with minimal supervision.
• Excellent observation, problem solving, verbal and written communication skills.
• Ability to organize and prioritize workload independently.

Responsibilities

• Develop and initiates the plan of care with the patient, caregiver(s) and physician.
• Initiates nursing procedures as ordered by the physician and in the plan of care.
• Provides patient, caregiver, and family counseling and education.
• Prepares clinical notes and communicates with the home health physician as needed.
• Identifies discharge planning needs and supports transitions home.
