Providence Life Services’ Providence Hospice seeks a compassionate RN Hospice Case Manager to provide skilled nursing and comprehensive case management for patients at home, in nursing facilities, or other settings in St. John, IN and Lake County.

This full-time role includes an on-call night shift weekly and a $5,000 retention bonus, supporting a dedicated team delivering dignified, patient-centered end-of-life care.

Qualifications

• Active RN license in Indiana.
• At least 1 year RN experience, preferably in Hospice or Home Health.
• Driver's license in good standing.
• Reliable transportation.

Responsibilities

• Provide skilled nursing care.
• Provide case management services.
• Conduct initial assessments of new patients.
• Travel to patients wherever they reside (home, nursing facility, etc.).
