Job summary

Gentiva is seeking a Hospice RN / RN Case Manager to serve patients in home settings across Texas. You will guide families through the hospice journey with compassionate, personalized care.

You will perform initial and ongoing assessments, document plans, and coordinate with physicians, LPNs, CNAs, social workers and chaplains to ensure comprehensive support. This role emphasizes caregiver education and timely communication.

Qualifications

• RN license in state of employment
• 1 year RN experience in hospice, palliative care, or related setting preferred
• CPR certification and valid driver's license

Responsibilities

• Perform comprehensive patient assessments and reassessments in home or facilities
• Deliver skilled nursing interventions and symptom management
• Develop and update individualized plans of care and visit frequency
• Coordinate care with physicians and IDG members
• Educate patients, families, and caregivers on disease, care needs, and medications
• Communicate changes in condition to the care team
• Maintain accurate clinical records and follow infection control standards
• Support continuity of care across home health, hospice, and palliative services
