Job summary

Swan Hospice in New Jersey is actively seeking Registered Hospice Nurses (RN) to join our team in full- or part-time roles. Nurses provide skilled nursing care in skilled nursing facilities, assisted living facilities, and private homes, adhering to physician orders and Medicare/state requirements.

As a member of the Interdisciplinary Team, you will assess patients, develop and adjust care plans, educate families, supervise aides, document care, and participate in care conferences.

Qualifications

• Graduate of an accredited registered nursing program.
• RN license in good standing for New Jersey.
• 1+ year of hospice, home health, or related nursing experience preferred.
• CHPN certification preferred.
• Bachelor's degree in Nursing preferred.

Responsibilities

• Provide comprehensive nursing care to hospice patients per physician orders.
• Lead and participate in IDT planning and care conferences.
• Educate patients and families about care plans and medications.
• Supervise and mentor Hospice Aides/Homemakers.
• Document care accurately and maintain records.
