Job summary

Qualifications

• Must be a Graduate of an accredited school of nursing
• Registered Nurse with a license in good standing
• Minimum 1 year of professional nursing experience in the last 3 years (public health/home care/hospice preferred)
• Must be able to lift and transfer patients and work in home settings

Responsibilities

• Performs comprehensive patient and family assessments to determine hospice needs and plans
• Provides professional nursing care per the nursing process
• Develops and revises plans of care with patient and family involvement
• Initiates and updates the care plan as patient needs change
• Uses assessment data to determine nursing diagnoses and actions
• Delivers patient education and self-care instruction
• Coordinates care with physician and interdisciplinary team
• Maintains accurate clinical notes and records
• Ensures safe patient environments and continuity of care
• Participates in on-call duties and equipment coordination
• Supervises ancillary personnel when required
