Gentiva is seeking an experienced Hospice RN / RN Case Manager to deliver comprehensive, patient-centered end-of-life care in patients' homes and facilities in Indiana. You will assess, plan, implement, and document nursing care while coordinating with physicians, social workers, and other care team members to ensure comfort and dignity.

Qualifications

• Active nursing license in the state of employment (Indiana).
• Minimum 1 year RN experience in acute care, oncology, long-term care, hospice or home health.
• Hospice or palliative care experience preferred.
• CPR certification.
• Current automobile insurance and valid driver's license.

Responsabilités

• Be the primary point of contact for patients and families, guiding them through the hospice journey.
• Deliver and document skilled nursing care per each patient’s Plan of Care, in home or other settings.
• Perform initial and ongoing assessments; monitor condition and adjust care as needed.
• Collaborate with the interdisciplinary team to build and refine care plans.
• Provide emotional, physical, and spiritual support to patients and families; educate and reassure caregivers.
• Communicate patient condition updates to the care team for timely interventions.
• Maintain accurate clinical documentation and adhere to infection control standards.
• Support quality improvement and scheduling initiatives to ensure compassionate, efficient care.
• Assist patients and families in understanding hospice services, symptom management, and end-of-life processes.
