SouthernCare in Indiana is seeking a dedicated Hospice RN / RN Case Manager who will serve as the primary contact for patients and families, guiding them through their hospice journey and delivering hands-on nursing care in home settings or partner facilities.

You will perform assessments, coordinate with an interdisciplinary team, teach caregivers, and maintain accurate clinical documentation while following regulatory standards.

Formación

• Current license to practice nursing in Indiana
• Hospice or palliative care experience preferred
• Minimum of one year experience as an RN in acute care hospital, hospice, home health or related setting

Responsabilidades

• Perform comprehensive patient assessments and reassessments, providing education to patients, families, and caregivers as appropriate
• Deliver skilled nursing interventions and symptom management per plan of care
• Develop and update individualized plans of care including visit frequency
• Coordinate care across interdisciplinary teams
• Educate patients and caregivers on disease process, self-care, end-of-life care, medications and nutrition
• Communicate changes in condition to the care team for timely interventions
• Support continuity of care across home health, palliative and hospice services
• Document timely clinical records including orders, progress notes, and care plans
• Maintain strong IDG relationships and involve team as needed
• Participate in on-call and patient support activities as applicable
