Job summary

Chapters Health is seeking an RN, Case Manager to assess patient and family needs, coordinate the Plan of Care with the IDT, and deliver clinical and palliative care to help patients stay in their homes as long as possible.

Requirements include an active RN license in Florida, at least 1 year of hospice or hospital experience, and BLS certification. EMR experience and reliable transportation are also expected.

Qualifications

• Current license as RN in the state where the employee will be working.
• Minimum of one (1) year nursing experience in hospice or hospital experience preferred.
• Active BLS for healthcare professionals from the American Heart Association or Red Cross.
• Experience with EMR/EHR systems.

Responsibilities

• Provides and manages direct care to patients and families as part of Interdisciplinary Team (IDT).
• Coordinates Plan of Care reflecting patient's evolving needs.
• Educates patient and family about disease, palliative care, and safety practices.
• Home visits to assess safety, medications, nutrition, and DME.
