Job summary

Chapters Health System seeks an RN Case Manager to assess patient and family needs, coordinate the Plan of Care with the IDT, and deliver clinical and supportive care to help patients remain in their homes as long as possible.

Qualifications

• RN license in the state where the employee will be working.
• Minimum of one year nursing experience in hospice or hospital setting preferred.
• BLS certification from American Heart Association or Red Cross.
• Experience using an EMR/EHR system.
• Valid driver’s license and reliable transportation per policy.

Responsibilities

• Provides and manages direct care to patients and families as part of the IDT.
• Coordinates the Plan of Care reflecting patient’s evolving needs.
• Educates patients and families about disease, care, and safety practices.
• Document patient information in the EMR and communicate with IDT.
