Job summary

Requires Florida RN license, hospice or hospital experience preferred, and a valid driver’s license.

Qualifications

• Current license as RN in Florida.
• Minimum of one year nursing experience in hospice or hospital setting preferred.
• Experience with EMR/EHR systems.

Responsibilities

• Provides and manages direct care to patients and families as part of the IDT.
• Coordinates the Plan of Care reflecting patient’s evolving needs.
• Acts as liaison with physicians and IDT to ensure timely communication and care.
