Job summary

Chapters Health in Florida seeks a dedicated RN Case Manager to assess patient and family needs, coordinate the Plan of Care with the Interdisciplinary Team (IDT), and provide clinical, palliative and supportive care to keep the participant in their home environment as long as possible.

The role requires a current Florida RN license, hospice or hospital experience preferred, EMR experience, valid driver's license, reliable transportation, and strong English communication.

Qualifications

• Current Florida RN license is required.
• Minimum one year nursing experience in hospice or hospital setting preferred.
• BLS for healthcare professionals from AHA/Red Cross.

Responsibilities

• Assess patient/family needs and coordinate the Plan of Care with the IDT.
• Provide clinical, palliative and supportive care to keep patients at home.
• Document care and communicate with physicians and IDT.
