Job summary

Capital Caring Health is seeking a compassionate RN Case Manager to assess needs, coordinate care with an interdisciplinary team, and deliver clinical and palliative support in patients' homes. You will ensure plans reflect evolving needs and maintain safety and dignity for patients and families.

The role requires an active RN license, hospice/hospital nursing experience, EMR proficiency, a valid driver's license, and strong English communication skills. Travel to patient homes is essential.

Qualifications

• Current RN license in the working state.
• Hospice or hospital nursing experience preferred.
• Alzheimer's/related training or certification may be required.
• Experience with EMR/EHR systems.
• Valid driver's license and reliable transportation.
• Effective verbal and written English communication.

Responsibilities

• Assess patient/family needs and coordinate the Plan of Care with the IDT.
• Provide clinical, palliative and supportive care in home settings.
• Obtain physician orders and document per policy.
• Communicate with physicians and IDT; SBAR as needed.
• Visit participants in their homes; manage scheduling and safety.
• Respond to emergencies and adapt care plans accordingly.
