Qualifications

• Current RN license in the state of employment.
• At least 1 year of nursing experience in hospice or hospital setting preferred.
• Experience with EMR/EHR systems and home visits in patient care.
• Valid driver’s license and automobile insurance; reliable transportation.
• Ability to communicate effectively in English, both verbal and written.
• BLS for healthcare professionals from AHA or Red Cross.

Responsibilities

• Provide direct patient and family care as part of the IDT.
• Coordinate Plan of Care and conduct initial/semi-annual assessments.
• Educate patients and families about disease, prevention, and palliative options.
• Communicate findings with physicians, IDT members and supervisors using SBAR.
• Visit patients at home, ensure safety, and manage documentation in EMR.
