Job summary

Capital Caring Health in Arlington, VA is hiring an RN, Case Manager to assess patient and family needs, coordinate the Plan of Care with the IDT, and deliver clinical, palliative and supportive care at home to keep patients in their homes as long as possible.

This role requires an active RN license in the assigned state, at least 1 year of nursing experience ( hospice or hospital preferred), and BLS certification. A $10,000 sign-on bonus is offered and benefits begin on day one.

Qualifications

Responsibilities

• Provide and manage direct patient and family care as part of the Interdisciplinary Team (IDT).
• Coordinate the Plan of Care to reflect evolving patient needs.
• Educate patients and families about disease progression, palliative care, and safety practices.
• Conduct home visits and assess home safety, medication and adherence to care plans.
• Document and communicate findings with IDT and attending physicians.
