Job summary

Choice Hospice, Inc. is seeking a Hospice RN Case Manager to deliver compassionate, patient-centered care to individuals at home or in related settings.

You will assess needs, coordinate nursing care, and work with the IDG to ensure comfort and dignity for patients and support for families. The role requires strong clinical skills, clear communication, and the ability to make independent decisions in dynamic environments; Hospice RN experience is preferred and HomeCare HomeBase experience is a

Qualifications

• Current RN license to practice in Colorado.
• CPR certification required.
• Valid driver's license and auto insurance.
• At least 2 years of Hospice RN experience preferred.
• HomeCare HomeBase experience preferred.

Responsibilities

• Perform initial and ongoing comprehensive assessments of patient needs in home or care settings.
• Develop and implement the plan of care coordinating with IDG for quality and continuity.
• Educate patients and families about disease processes, end-of-life care, nutrition and self-care.
• Provide on-call services and respond to changes in patient needs.
• Communicate with physicians and other team members about patient progress.
